Community staff attached to the Knoll

We are a test bed site for a new way of working between the Community staff (you will recognise District Nurses) we have not quite got a name for it yet but we think of it as the Integrated Team.  This is bringing together both health and social services.  The team consists of District Nurses, Community Matrons, Physiotherapists, Re-ablement specialists, occcupational Health Specialists and representatives from Social Services.  The team targets the over 75's at the moment and we aim to improve the provision of all styles of home services and act as a central point for care outside hospital.  Indeed one of the important tasks for the team is to 'reach in' to hospital and ensure everything is in place for the sucessful return to home for patients.

 

The District Nurses 

The district nursing team are based at the surgery and are available to help patients who need nursing assistance at home. Visits can be arranged directly with the nurses via the surgery, or they may be requested by the practice doctors or in liaison with the hospital following discharge.

The Health Visitors

can give advice on health care, particularly for expectant and nursing mothers, small children, the elderly and the disabled. Our Health Visitors are specially trained to help and advise patients with heart disease. Their telephone number, which is also an answerphone, is: 01928 732348

The Community Midwives

Iona Clarkson can be contacted on 01244 365166 (Community Office at The Countess of Chester Hospital) or at The Knoll Surgery on Tues/Thurs afternoons.

Counselling Services

The Primary Care Trust provide a number of counsellors covering many different areas where patients may require this form of support.
